Referral Forms

If you are a DCBS Worker and are making a referral for a family to receive a CATS Assessment, please use this form. Keep the completed form in MS Word format and email to CATSReferrals@uky.edu. When submitting, please include the Court Order (if applicable) and the TWIST Case Summary for the family (accessed by clicking the case number in TWIST). If you have any questions regarding making a CATS Referral, please call us at 859-218-6901.

If you are making a referral for a child to receive trauma therapy, please download the form below. Email the completed form to CATTTIReferrals@uky.edu or fax to 859-543-1121. If you have any questions regarding making a CATTTI Referral, please call the CATTTI office at 859-218-6956.
