CATS Referral
If you are a DCBS Worker and are making a referral for a family to receive a CATS Assessment, please use this form. Keep the completed form in MS Word format and email to CTAC@lsv.uky.edu. When submitting, please include the Court Order (if applicable) and the Case Summary Face Sheets. If you have any questions regarding making a CATS Referral, please call us at 859-218-6901.
CATTTI Referral
If you are making a referral for a child to receive trauma therapy, please download the form below. Email the completed form to Kristal.Hankinson@uky.edu or fax to 859-543-1121. If you have any questions regarding making a CATTTI Referral, please call the CATTTI office at 859-218-6956.
Early Childhood Mental Health Provider Consultation Referral
